This page provides registration dates, fees, and forms for MYSA soccer.  Registration can be completed in person on designated registration dates (at Crestwood High School) or by mail.

Registration dates for the MYSA fall, 2017 soccer season were held at Crestwood High School in March and April.

(Use this form to register any player who was NOT registered to play soccer with MYSA last year.  Be sure to provide a photocopy of the player's birth certificate.)

About our Annual Fund Raiser


At registration, you'll be asked to participate in our annual fund raiser.  Here's how it works:


MYSA is a non-profit, charitable, 501(C)3 organization, dedicated to providing the equipment, facilities and services needed for its youth soccer program here in Mountain Top.

Our goal is to fund our programs and facilities through a combination or registration fees and fund-raisers.

Our fund raiser allows several options for each family:

1.)  Purchase 12 raffle tickets from MYSA for $60, sell them for $5 each, keep the proceeds and thereby break even.

2.)  Purchase 24 raffle tickets from MYSA for $90, sell them for $5 each, keep the proceeds and thereby earn $30 (in effect reducing the out-of-pocket registration cost by $30).

3.)  Opt out of the fund raiser entirely by paying to MYSA an opt-out fee of $30.

We are sometimes asked what our registration and fund raiser fees are used for. Here are some, but probably not all, of the expenses MYSA incurs each year:

Equipment (goals, nets, balls, corner flags)

Uniforms for each player, including shirt, shorts and socks

Paint lining machines

Paint to line fields

At the Soccer Complex

     Property and liability insurance

     Turf maintenance (mowing, seeding, fertilizing)

     Road maintenance

     Irrigation system maintenance

     Electrical service bills


     Concession Building maintenance

Programs

     Referee fees

     Registration and insurance fees paid to EPYSA


     Registrations fees paid to Wyoming Valley Rec League
