Please fill out one form per child.
While filling out the registration form, fields marked with a * are required. Please be sure to fill out the field that says "Any known Allergies / Medical Conditions to consider?*" If there are no medical conditions, please type "None" or "N/A".
Players born between Jan. 1, 2007 - Dec. 31, 2018 are eligible.
Registration deadline is May 31, 2022.
Registration Fee will be $50 per player.

We are partnering once again with Bethany Soccer Camps to offer soccer clinics August 7-9, 2022. If you would like to sign your child up now, please choose the option on the form. You will be charged an additional amount of either $65 (for U6) or $95 (for U8 - U15).
